As far as I know, there's no FORMS library available for the DEC Alpha.
(There's been talk of porting FORMS to Open GL, which I think is available
on the Alpha, but don't think that's happened yet.)

Daeron Meyer has written several geomview modules in Motif, though.
If you grab the geomview source distribution,
   pub/software/geomview/geomview-src.x11-beta.tar.Z
from geom.umn.edu, it includes sources to those modules.

Also, some people are beginning to write geomview modules in Tcl/Tk.
